Please bring your insurance information to each visit. Payment is expected and required at the time of service unless prior arrangements have been made. You may pay with cash, check, money order, Visa or MasterCard. Health insurance is a contract between you and your carrier; it is your responsibility to know and understand the coverage that it provides. SFMA participates with most health plans. If you do not have insurance coverage, we offer discounted rates for payment at the time of service.

There are a variety of appointment options for you to choose from so you can get help when you need it.
If you are late for your scheduled appointment, your appointment may have to be rescheduled out of consideration for our other patients who have appointments.
Missed appointments / Cancellations:
Broken appointments represent a cost to us, to you, and to our other patients who could have been seen in the time set aside for you. Cancellations are requested 24 prior to the appointment. Excessive abuse of scheduled appointments may result in discharge from the practice.
Superior Family Medical is concerned with the health of our patients. We believe in the necessity of preventative care as well as adequate assessment and follow-ups on medical problems, and continued monitoring of medications. We strive to accomodate patients in a timely manner. If you are unable to keep your appointment for any reason, we request notification in advance of your scheduled appointment time so that others may be seen in that timeslot.

These must be filled out and returned to our office before any medical records will be processed. Medical records are worked on every Thursday. There may be a fee for the copying of medical records; you will be informed of this fee at the time of your medical records request.
